38% urban Indians consume artificial sweeteners every month, survey finds

A recent survey conducted by Local Circles in India found that 38% of urban Indians consume artificial sweeteners every month. In May 2023, the World Health Organization (WHO) warned against the use of artificial sweeteners, stating they increase the risk of type 2 diabetes and cardiovascular diseases. The International Agency for Research on Cancer (IARC) has not concluded that there is definitive proof of a link between aspartame and liver cancer.

Following the buzz around the World Health Organization (WHO) stating aspartame to be categorized as “possibly carcinogenic to humans”, a recent survey by Local Circles found that 38 per cent urban Indians consume artificial sweeteners every month.

As many as 11,342 people responded to the question of which 38 per cent people confirmed their consumption of artificial sweeteners in food every month.

Aspartame is a popular artificial sweetener used in food products like diet soda, sugar-free gums, etc. Apart from this, the Indian food regulatory body, Food Safety and Standards Authority of India (FSSAI), has allowed the use of five other artificial sweeteners.

These include saccharin sodium, aspartame, acesulfame potassium, sucralose, neotame, and isomaltulose.

The survey which received as many as 23,000 responses found that 1 in 3 urban Indians consuming artificial sweeteners, consume diet soda or chewing gum.

Several respondents of the total of 4,310 consumers who responded to this query indicated consumption of these sweeteners in more than one food product.

Besides diet soda and chewing gum, both being consumed by 32 per cent of the respondents, other food products were also found to be in rising trends.

As many as 26 per cent people indicated consumption of energy drinks, while 18 percent consume sugar free chocolates. Meanwhile, 18 per cent of consumption was recorded for sugar free ice creams, 5 per cent for energy bars and 37 per cent indicated other foods and drinks.

The International Agency for Research on Cancer (IARC), WHO’s cancer research arm, has not ruled that there is conclusive proof of a link between aspartame and liver cancer, as suggested by some research papers.

In May 2023, the global health body, WHO warned against artificial sweeteners stating that these products are not helpful in losing weight. In fact, the consumption of these increases the risk of type 2 diabetes and cardiovascular diseases.

After a meeting of the IARC’s external experts, the cancer research body’s ruling in June is based on all the published evidence thereby ending confusion among the public over varied reports.

However, 90 per cent of the respondents said they want food brands to display the use of artificial sweeteners prominently on the front of the pack.

Several food and beverage brands, who have major consumers as minors, use artificial sweeteners like aspartame, sucralose and steviol glycoside in their energy drinks, gums, ice creams etc.

Few also say zero sugar on their packaging.

“By the health regulator at least making it mandatory for all such products to carry prominently on the front of the pack the fact that the product has artificial sweeteners, at least parents will be able to make an informed decision,” Local Circles said in its report.

Dengue is a viral disease transmitted by the infective bite of a mosquito called Aedes aegypti. Human beings develop disease after 5–6 days of being bitten by infected female mosquitoes that feed both indoors and outdoors during the daytime (from dawn to dusk). These mosquitoes thrive in areas with standing water including puddles, water tanks, containers and old tires. It occurs in two forms – classical dengue fever also known as ‘break bone’ fever and dengue haemorrhagic fever (DHF) which is life threatening. It is highly contagious and spreads from one person to another through mosquito bites,” says Dr Tushar Tayal, Consultant- Internal Medicine, CK Birla Hospital, Gurugram.

“The infection is characterized by flu-like symptoms which include a sudden high-grade fever coming in separate waves, pain behind the eyes, muscle, joint and bone pain, severe headache, abdominal fullness and discomfort and a skin rash with red spots which happens by the fifth day of illness,” says Dr Tayal.

Dengue illness can be prevented by using basic steps of hygiene and source control to ensure and eliminate any possible breeding locations in your home, such as watering cans, buckets, and clogged drains.

Signs and symptoms of dengue explained by Dr Panda

1. High fever: One of the primary signs of dengue fever is a sudden onset of high fever, often reaching 104°F (40°C). The fever typically lasts for 2 to 7 days.

2. Severe headache: Dengue fever is often accompanied by an intense headache, especially behind the eyes. This headache can be debilitating and persistent.

3. Pain behind the eyes: Individuals with dengue fever may experience pain and discomfort behind their eyes. This symptom is often described as a deep ache.

4. Joint and muscle pain: Dengue fever is known for causing severe joint and muscle pain, earning it the nickname ‘breakbone fever.’ The pain can be widespread and excruciating.

5. Rash: A characteristic rash may develop on the skin after a few days of fever. It typically appears as small, red, and raised spots and can be itchy.

6. Nausea and vomiting: Dengue fever can cause gastrointestinal symptoms such as nausea, vomiting, and abdominal pain. These symptoms may contribute to dehydration.

7. Fatigue: Individuals affected by dengue often experience extreme fatigue and weakness, even after the fever subsides. This fatigue can persist for weeks.

8. Bleeding: In some cases, dengue fever can lead to bleeding manifestations, such as nosebleeds, bleeding gums, or easy bruising. If severe, it may indicate dengue hemorrhagic fever, a potentially life-threatening condition.

Myths associated with dengue

“More than this illness, myths and panic associated with dengue is viral. The majority (90%) of people recover from this viral infection without much medical intervention following simple dictum of preventing dehydration by drinking with plenty of water, ensuring rest and relief of fever and pain with paracetamol. Severe/complicated dengue is seen in only about 10 per cent patients and only 5 per cent of dengue patients need to be hospitalised,” says Dr Panda.

“It is essential to note that dengue symptoms can vary from person to person, and some individuals may have mild or no symptoms at all. However, anyone experiencing the aforementioned symptoms, especially in areas with a high prevalence of dengue, should seek medical attention promptly,” concludes Dr Panda.

Full provisions of medical devices regulation to come into force from October 1: DCGI

Speaking to reporters on the sidelines of the 9th International Pharmaceutical Exhibition (iPHEX) being organised by the Pharmaceuticals Exports Promotion Council (Pharmexcil) here, Raghuvanshi said the regulations would ensure quality in the production of medical devices. “In the medical devices, we are categorising into four groups. Two groups (A and B) are already notified. C and D are left which will happen from October 1,” he said.

Medical devices of all four categories (A, B, C, and D) will be brought under the regulation by October 1 and as of now only two groups (A and B) are being regulated, Drugs Controller General of India (DCGI) Rajeev Singh Raghuvanshi said on Wednesday. Speaking to reporters on the sidelines of the 9th International Pharmaceutical Exhibition (iPHEX) being organised by the Pharmaceuticals Exports Promotion Council (Pharmexcil) here, Raghuvanshi said the regulations would ensure quality in the productivity in the production of medical devices.

“In the medical devices, we are categorising into four groups. Two groups (A and B) are already notified. C and D are left which will happen from October 1,” he said.

The Union Cabinet on April 26 approved the National Medical Devices Policy, 2023 to give a thrust to the sector with quality controls. The market size of the medical devices sector in India is estimated to be USD 11 billion (approximately Rs 90,000 crore) in 2020 and its share in the global medical device market is estimated to be 1.5 per cent, an official release had earlier said.

Earlier during the inaugural session of iPHEX, S V Veeramani, Chairman of Pharmexcil, highlighted that the Indian pharmaceutical industry has carved out a unique distinction in reaching 200 countries in the world.

He said the Indian pharma exports were pegged at USD 25.39 billion in FY 23 and the target for the current year is USD 28 billion.

Ravi Udaya Bhaskar, Director General of Pharmexcil, said the Indian pharmaceutical industry has contributed substantially to healthcare through the supply of quality, affordable generic medicines to nearly 200 countries and is popularly called as the ‘Pharmacy of the World’.

Bhaskar further said earlier all the eight editions attracted over 4,500 foreign delegates, including regulatory officials, public procurement agencies, journalists and importers from more than 120 countries, 3,500 exhibitors and one lakh visitors.

Analysing Covid-patients’ blood samples, researchers found that 3 months after being hospitalised for severe infection, there was no rapid increase in the immune cells targeting SARS-CoV-2, but an increase in those targeting the dormant Cytomegalovirus (CMV). CMV is usually harmless but can stay in your body for life once infected with it.

The finding indicated that the prolonged inflammation at 3 months in severe patients may not be driven by SARS-CoV-2 but instead may be “bystander driven”.

“Long Covid occurs in one out of ten COVID-19 cases, but we still don’t understand what causes it.

“Several theories proposed include whether it might be triggered by an inflammatory immune response towards the virus that is still persisting in our body, sending our immune system into overdrive or the reactivation of latent viruses such as human cytomegalovirus (CMV) and Epstein Barr virus (EBV),” said Laura Rivino, senior lecturer at the University of Bristol, UK, and the study’s lead author.

The researchers analysed blood samples of 63 patients hospitalised for mild, moderate or severe COVID-19 infection at the start of the pandemic when vaccines were not available.

The study also found patients with severe COVID-19 to display a higher number of long Covid symptoms compared to mild and moderate patients, even as the team’s analysis revealed no direct association between long Covid symptoms and Covid-related immune inflammatory responses.

Further, 79 per cent of all the patients reported at least one ongoing long Covid symptom with breathlessness and excessive fatigue being the most common, the study published in the journal eLife said.

The significant dysfunction found in the immune profiles of severe patients at 3 months indicated that inflammation may persist even for months post-recovery, even though results showed the inflammation in these patients to resolve over time.

At 12 months, the researchers found, both the immune profiles and inflammatory levels of patients with severe disease were similar to those of mild and moderate patients.

“Our findings suggest that prolonged immune activation and long Covid may correlate independently with severe COVID-19.

“Larger studies should be conducted looking at both a larger number of patients, including if possible vaccinated and non-vaccinated COVID-19 patients, and measuring a larger range of markers and cytokines.

“Understanding whether inflammation and immune activation associate with long Covid would allow us to understand whether targeting these factors may be a useful therapy for this debilitating condition,” said Rivino.

Soon you may need doctor’s prescription for cough syrups

The Drugs Technical Advisory Board (DTAB), India’s apex drug advisory body, in its meeting last month was also considering a proposal for exempting dried Ivy leaf extract sugar free cough syrup from the requirements of a prescription.

The government is considering whether cough syrups should be sold on the prescription of a registered medical practitioner (RMP) only. A sub-committee of experts has been constituted to examine the requirement of RMPs for the sale of various cough syrups in the country, people in the know told ET.

Cough syrups are widely available over the counter as of now.

The Drugs Technical Advisory Board (DTAB), India’s apex drug advisory body, in its meeting last month was also considering a proposal for exempting dried Ivy leaf extract sugar free cough syrup from the requirements of a prescription.

However, after deliberating the matter, the DTAB recommended “that a sub-committee should be constituted to examine the requirement of RMP for sale of various cough syrups in the country,” the minutes of the meeting said.

The sub-committee will also examine the proposal to exempt dried Ivy leaf extract cough syrup from the warning “to be sold by retail in the prescription of RMP only.”

India-made cough syrups have been under the scanner in the recent past with many countries linking it with deaths of children in their countries.

He advised, “Vulnerable individuals must be aware of the symptoms and signs of cancer so that they can seek early medical intervention. Some common symptoms and warning signs like unexplained weight loss, fatigue, changes in skin colour, persistent pain, changes in bowel or bladder habits, unusual bleeding, difficulty swallowing or persistent indigestion, changes in a cough or hoarseness, lumps or thickening are associated with cancer and could indicate malignancies. These abnormalities can occur at any age and not gender-specific. Regular check-ups and screenings can help detect cancer early, when there is a high likelihood of a favourable response to treatment.”

Bringing his expertise to the same, Dr (Prof) Vijay Patil, Consultant – Medical Oncology at PD Hinduja Hospital and MRC in Mumbai, echoed that cancer is a disease that can occur at any time or age, though it is commonly known to affect older individuals. He said, “It is important to understand that cancer does not discriminate based on age, gender or lifestyle choices. While certain factors such as exposure to harmful chemicals, smoking and poor dietary habits can contribute to the development of cancer, there are cases where people with no known risk factors have also been diagnosed with the disease. Early detection and regular screening are crucial in detecting cancer in its early stages, regardless of age, and can significantly improve outcomes for patients.”

He too highlighted that the common symptoms and warning signs of cancer include unexplained weight loss, persistent fatigue, changes in bowel or bladder habits, difficulty swallowing, persistent coughing or hoarseness, unusual bleeding or discharge, lumps or thickening in the breast or other body parts, unexplained pain and fever. He concluded, “It is important to note that these symptoms are not exclusive to cancer and could also be due to other medical conditions. If you experience any of these symptoms for an extended period, it is recommended that you see a healthcare professional for further evaluation. Early detection and treatment can improve the chances of successful cancer treatment.”

Eye drops slow nearsightedness progression in children: Study

The study shows that daily drop of low dose of atropine in each eye is better than placebo at limiting eyeglass prescription changes.

According to the findings of recent clinical research, the first medication therapy to reduce the onset of nearsightedness in children may be on the horizon.

The three-year study found that a daily drop in each eye of a low dose of atropine, a drug used to dilate pupils, was better than a placebo at limiting eyeglass prescription changes and inhibiting elongation of the eye in nearsighted children aged 6 to 10.

That elongation leads to myopia, or nearsightedness, which starts in young kids and continues to get worse into the teen years before leveling off in most people. In addition to requiring life-long vision correction, nearsightedness increases the risk for retinal detachment, macular degeneration, cataracts and glaucoma later in life – and most corrective lenses don’t do anything to stop myopia progression.

“The idea of keeping eyeballs smaller isn’t just so people’s glasses are thinner – it would also be so that in their 70s they don’t suffer visual impairment,” said lead study author Karla Zadnik, professor and dean of the College of Optometry at The Ohio State University.

 

“This is exciting work for the myopia research community, which I’ve been part of for 35 years. We’ve talked about treatment and control for decades,” she said. “And it’s exciting to think that there could be options in the future for millions of children we know are going to be myopic.”

The results of the CHAMP (Childhood Atropine for Myopia Progression) trial are published today (June 1, 2023) in JAMA Ophthalmology.

Eye drops slow nearsightedness progression in children: Study

About one in three adults worldwide is nearsighted, and the global prevalence of myopia is predicted to increase to 50% by 2050. Though one federally approved contact lens can slow progression of nearsightedness, no pharmaceutical products are approved in the United States or Europe to treat myopia.

Animal studies years ago hinted at atropine’s ability to slow the growth of the eye, but the full-strength drug’s interference with near vision and concerns about pupil dilation hindered early considerations of its potential as a human therapy for myopia. More recent research has suggested a low dose of atropine might be the ticket.

This new double-masked, randomized phase 3 trial assessed the safety and effectiveness of two low-dose solutions, with atropine concentrations of either .01% or .02%, versus placebo. Treatment for each of the 489 children aged 6 to 10 assessed for the drug’s effectiveness consisted of one daily drop per eye at bedtime, which minimized the disruption of any blurring effects atropine might have on vision.

Researchers were a bit surprised to find that the most significant improvements at all time points compared to placebo resulted from the solution containing .01% of atropine. Though the .02% atropine formulation was also better at slowing progression of myopia than placebo, the results were less consistent.

“The .01% story is clearer and more obvious in terms of significantly slowing both the growth of the eye as well as then resulting in a lower glasses prescription,” Zadnik said.

Including a measure of the eye’s growth was a key component of the study because “the field is actually moving toward axial elongation being as important as, or more important than, the glasses prescription in terms of the most meaningful outcome,” she said. “If we’re trying to slow eye growth to prevent bad outcomes for people in their 80s, measuring the eye growth directly is really important.”

The drugs’ safety was assessed in a larger sample of 573 participants that also included children as young as 3 and up to age 16. Both low-dose formulations were safe and well tolerated. The most common side effects were sensitivity to light, allergic conjunctivitis, eye irritation, dilated pupils and blurred vision, although reports of these side effects were few.

The CHAMP trial was the first study of low-dose atropine to include placebo controls for three years and to involve a large, diverse population recruited from 26 clinical sites in North America and five countries in Europe. In a second section of the trial, researchers are evaluating how the eyes respond when the treatment is over.

The experimental drug is made without preservatives and, if federally approved as a therapy, would be distributed in single-use packaging for convenience and to prevent contamination. Off-label low-dose atropine that can currently be obtained at compounding pharmacies may contain preservatives that can lead to dry eye and corneal irritation, researchers noted.

The experimental product studied in the CHAMP trial is manufactured by Vyluma, a New Jersey development-stage biopharmaceutical company with a focus on pharmaceutical treatments for refractive errors of the eye. A subsidiary of Nevakar Inc., Vyluma sponsored the trial, has submitted a New Drug Application to the FDA to seek approval in the U.S. and has partnered with two companies to commercialize the product outside the United States..

Zadnik led the study as a paid expert consultant to Vyluma. Jennifer Fogt, assistant professor at Ohio State, also worked on the trial. Additional co-authors Erica Schulman of SUNY College of Optometry; Ian Flitcroft of the Center for Eye Research in Dublin, Ireland; Louis Blumenfeld of Eye Physicians of Central Florida; and Tung Fong, Eric Lang, Houman Hemmati and Simon Chandler of Vyluma represented the CHAMP trial group investigators.

Conflict, climate change, Covid, cost-of-living heighten threats for most vulnerable women, babies: UN report

The report sounds the alarm on a silent emergency of preterm birth which is impeding progress in improving children’s health and survival.

An estimated 13.4 million babies were born preterm in 2020, with 45 per cent of them being born in just five countries, including India, China and Pakistan, according to a report released by the United Nations agencies and partners on Wednesday. The report titled ‘Born Too Soon: Decade of Action on Preterm Birth’ is developed by more than 140 individuals from 46 countries and is produced by the World Health Organisation (WHO) and the United Nations Children’s Fund (UNICEF) together with PMNCH — the world’s largest alliance for women, children, and adolescents.

The report sounds the alarm on a “silent emergency” of preterm birth which is impeding progress in improving children’s health and survival.

The global preterm birth rate was 9.9 per cent in 2020, compared to 9.8 per cent in 2010. An estimated 13.4 million babies were born pre-term in 2020, with nearly 1 million dying from preterm complications, which is equivalent to around 1 in 10 babies born early (before 37 weeks of pregnancy) worldwide.

In 2020, Bangladesh had the highest estimated preterm birth rate (16.2 per cent), followed by Malawi (14.5 per cent) and Pakistan (14.4 per cent). Rates are also high in high-income countries, such as Greece (11.6%), and the United States of America (10.0%), the report stated.

“Almost half (45 per cent) of all preterm babies in 2020 were born in just five countries: India, Pakistan, Nigeria, China and Ethiopia,” it said.

The report highlighted that preterm birth rates have not changed in the past decade in any region of the world and that “four Cs” – conflict, climate change, COVID-19 and the cost-of-living crisis – heighten threats for the most vulnerable women and babies in all countries.

For example, air pollution is estimated to contribute to 6 million preterm births each year.

Nearly 1 in 10 preterm babies are born in the 10 most fragile countries affected by humanitarian crises, according to a new analysis in the report. Climate science is increasingly exposing the direct and indirect effects of climate change on pregnant women, stillbirths and preterm birth.

According to it, 1.2 million babies are ‘born too soon’ in the 10 most fragile countries affected by humanitarian crises, where accessing care for women and babies is particularly challenging.

“More investment is needed to specifically mitigate risks and to increase focus on women and babies in policies and programmes addressing the climate emergency,” it said.

“Born too soon underlines why we must increase investment and accountability for preterm birth – the world’s largest cause of death of children under the age of five. Progress is flatlining for maternal and newborn health, as well as the prevention of stillbirths, and is now pushed back further through the devastating combination of COVID-19, climate change, expanding conflicts and rising living costs,” said Helga Fogstad, Executive Director, PMNCH.

“By working together in partnership – governments, donors, the private sector, civil society, parents, and health professionals – we can sound the alarm about this “silent emergency”, and bring preterm prevention and care efforts to the forefront of national health and development efforts, building human capital by supporting families, societies and economies everywhere,” Helga added.

With the report coming at a time when the International Maternal Newborn Health Conference (IMNHC) is being held at Cape Town from May 8, the WHO, UNICEF, UNFPA and PMNCH called for increased investments, accelerated implementation of established national policies for maternal and newborn care to improve care for women and newborns and mitigate risks from preterm births.

They also called for promoting education through the life cycle; supporting smarter economic investments, with co-financing across sectors; strengthening climate adaptation responses across the life course; and advancing the coordination and resilience of emergency systems, besides investing in locally-led innovation and research to support improvements in quality of care and equity in access.

Ensuring quality care for these tiniest, most vulnerable babies and their families is absolutely imperative for improving child health and survival, said Dr Anshu Banerjee, Director for Maternal, Newborn, Child and Adolescent Health and Ageing at WHO.

“Progress is also needed to help prevent preterm births — this means every woman must be able to access quality health services before and during pregnancy to identify and manage risks,” he said.

Long COVID linked with self-perceived cognitive deficits related to anxiety and depression

Experiencing persistent symptoms of COVID more than four weeks after initial infection is called Long COVID.

Symptomatic long COVID may be associated with self-perceived cognitive difficulties such as memory problems, according to a study. Long COVID is described as experiencing persistent symptoms of the disease more than four weeks after initial infection. Researchers at the University of California, Los Angeles (UCLA) in the US noted that over one in three people experiencing long COVID symptoms perceived such cognitive deficits, which have been found to be related to anxiety and depression.

The findings, published recently in the journal JAMA Network Open, show that psychological issues such as anxiety or depressive disorders may play a part in some people who are experiencing long COVID, technically known as post-COVID-19 condition, or PCC.

“This perception of cognitive deficits suggests that affective issues—in this case anxiety and depression—appear to carry over into the long COVID period,” said study senior author Neil Wenger, a professor at UCLA.

“This is not to say that long COVID is all in one’s head, but that it is likely not a single condition and that for some proportion of patients there is likely a component of anxiety or depression that is exacerbated by the disease,” Wenger said in a statement.

The researchers surveyed 766 patients who had confirmed symptomatic COVID infection and had either been hospitalised at UCLA or at one of 20 health care facilities in the US or were referred to the programme by a primary care physician and been treated as outpatients.

Long COVID linked with self-perceived cognitive deficits related to anxiety and depression

Patients were surveyed by telephone at 30 days, 60 days and 90 days following hospital discharge or, in the case of non-hospitalised patients, after the date of a positive COVID test to ascertain if they felt their health was back to normal.

The researchers found that 276 (36.1 per cent) of the patients surveyed perceived during the acute illness or the following weeks that they had cognitive difficulties.

The study also found that these patients were twice as likely as those without perceived cognitive deficits to report experiencing physical symptoms at 60 and 90 days.

The researchers acknowledged some limitations to the findings. These include a lack of objective cognition measures because the survey relied on subjective responses about cognitive deficits.

They also did not have data on participants’ possible cognition, depression, and anxiety prior to COVID infection.

The findings may not apply to other patient cohorts given that participants were treated at an academic medical centre and were referred to the programme based on physicians’ belief that the patients were at clinically high risk for cognition deficits, the researchers added.
