Covid-19: US health experts warn against taking ivermectin to treat coronavirus

Health experts and medical groups are pushing to stamp out the growing use of a decades-old parasite drug to treat COVID-19, warning that it can cause harmful side effects and that there’s little evidence it helps.

With the fourth wave of infections, more Americans are turning to ivermectin, a cheap drug used to kill worms and other parasites in humans and animals.

Federal health officials have seen a surge in prescriptions this summer, accompanied by worrying increases in reported overdoses. The drug was even given to inmates at a jail in northwest Arkansas for COVID-19, despite federal warnings against that use. On Wednesday, podcaster Joe Rogan, who has been dismissive of the COVID-19 vaccine, announced he had tested positive for the virus and was taking the medication.

Ivermectin has been promoted by Republican lawmakers, conservative talk show hosts and some doctors, amplified via social media to millions of Americans who remain resistant to getting vaccinated. It has also been widely used in other countries, including India and Brazil.

This week, the top U.S. professional groups for doctors and pharmacists appealed for an “immediate end” to the drug’s use outside of research.

“We are urging physicians, pharmacists, and other prescribers — trusted healthcare professionals in their communities — to warn patients against the use of ivermectin outside of FDA-approved indications and guidance,” said the American Medical Association and two pharmacist groups.

Large studies are now underway in the U.S. and overseas to determine if the drug has any effect on preventing or blunting COVID-19.

The latest plea follows similar warnings from federal and state regulators who are tracking side effects and hospital admissions tied to the drug.

Louisiana and Washington issued alerts after an uptick in calls to poison control centers. Some animal feed supply stores have run out of the drug because of people buying the veterinary form to try and treat COVID-19.

“There’s just not any good evidence right now suggesting this is a good treatment for treating or preventing COVID-19,” said Randy McDonough, a pharmacist in Iowa City, Iowa

Ivermectin is approved by the Food and Drug Administration to treat infections of roundworms and other tiny parasites in humans and animals like cows, horses, and dogs. Tablets are used for internal parasites while ointments are used to treat head lice and other skin infections. The generic drug works by paralyzing the worms and killing their offspring.

The FDA has tried to debunk online claims that animal-strength versions of the drug can help fight COVID-19.

“Taking large doses of this drug is dangerous and can cause serious harm,” the FDA warned in a public advisory. The drug can cause nausea, vomiting, diarrhea, seizures, delirium and even death, said the agency.

Dr. David Boulware of the University of Minnesota says the drug’s side effects are mild at two or even three times the usual human dose. But formulations for farm animals might contain 1,000 times what’s safe for humans.

“It’s pretty easy to get into toxic levels,” said Boulware, an infectious disease specialist. “All these concentrated doses that are meant for a 2,000-pound horse can certainly get people sick or hospitalized for toxicity.”

Boulware says he prescribes the drug to patients a few times a year in the U.S. and more routinely when working in countries where intestinal parasites are common. But he and other experts have been alarmed by the explosive growth in U.S. ivermectin prescribing.

By mid-August U.S. pharmacies were filling 88,000 weekly prescriptions for the medication, a 24-fold increase from pre-COVID levels, according to the Centers for Disease Control and Prevention. Meanwhile, U.S. poison control centers have seen a five-fold increase in emergency calls related to the drug, with some incidents requiring hospitalization.

The CDC citedone case of a man who drank an injectable form of ivermectin intended for cattle. He suffered hallucinations, confusion, tremors and other side effects before being hospitalized for nine days.

The World Health Organization, the National Institutes of Health and other medical experts have also recommended against using it outside of carefully controlled patient studies. An NIH panel found “insufficient evidence” for or against the drug for COVID-19, calling for more large, well-designed trials.

The experts noted that early laboratory research showed ivermectin slowed the replication of coronavirus when grown in monkey cells. But such studies are not useful for gauging real-world effectiveness in humans. And they noted other research suggesting the drug would need to be given at levels 100 times the standard dose to have antiviral effects in humans.

The NIH is studying the drug in a large trial comparing a half-dozen established drugs to see if they have some effect against COVID-19.

Govt’s e-Sanjeevani initiative Completes 1 Crore Consultations

The Union Ministry of Health & Family Welfare’s (MoHFW) National Telemedicine Service eSanjeevani has conducted over 1 crore teleconsultations across the country. “That eSanjeevani has been quickly adopted by patients, doctors, and specialists across the country is evident from the fact that it has shown astounding growth of over 1000% in the last 10 months. The National Telemedicine Service which was used for 160,807 teleconsultations in the month of September in 2020 enabled 1,650,822 teleconsultations in the month of July 2021,” MoHFW said in a statement.

It is noteworthy, MoHFW emphasized, that “even when the Internet penetration in the country is below 50%, this innovative digital health initiative of the Union Health Ministry has been able to defeat the tyranny of geography, distance, and time and establish itself as a parallel stream of healthcare services delivery.” eSanjeevani has also alleviated numerous challenges typically linked with healthcare delivery, such as higher costs and fatigue pertaining to seeking an in-person consultation with a doctor besides controlling the ongoing pandemic that had burdened the country’s healthcare infrastructure like never before, it added.

The health ministry had conceptualized the use of telemedicine to aid the implementation of Ayushman Bharat – the world’s largest government-owned health insurance scheme in and Family Welfare, in 2018. Accordingly, eSanjeevani, the flagship telemedicine technology of the government developed by the Mohali branch of the Centre for Development of Advanced Computing (C-DAC) was customized and eSanjeevaniAB-HWC was rolled out in 2019 as a doctor-to-doctor telemedicine platform. After the Covid-19 pandemic touched Indian shores, and all the OPDs in the country were closed because of the national lockdown, the ministry in sync with the release of Telemedicine Practice Guidelines by the government in March 2020 conceptualized another version of eSanjeevani for patient-to-doctor telemedicine. Accordingly, owing to the coordination between MoHFW and the eSanjeevani team in C-DAC Mohali, eSanjeevaniOPD was developed and deployed rapidly across the country. On 13 April 2020, eSanjeevaniOPD was launched to facilitate the provision of health services for patients in the confines of their homes.

IBM provides health facilities to 3 states to fight potential third wave

Technology firm IBM India has provided healthcare facilities like pediatric ICU beds and oxygen concentrators to Karnataka, Haryana and Uttarakhand which would help them tackle a potential third wave of COVID-19, according to senior company official.

The healthcare facilities have been provided as part of IBM India’s corporate social responsibility (CSR) initiative in partnership with the Nasscom Foundation.

Speaking to PTI, IBM India and South Asia Head-CSR Manoj Balachandran said, “While a lot of relief like oxygen cylinders was coming in to address the immediate short term need, we decided to work on mid to long term requirements with the state governments.”

In Karnataka, pediatric ICU beds were provided thinking that children could be a potential risk if there is a third wave as they are not vaccinated, he said.

“All that we are doing now is obviously trying to be prepared for the third wave, hoping against hope that the third wave is not going to be as severe as the earlier one,” he said.

Right now, the focus has been on these three states. But things might change if a third wave hits, as the company will then re-evaluate and look at other states for extending relief, he added.

According to IBM, 45 pediatric ICU beds have been provided to CV Raman General Hospital and Sevashram Hospital in Bengaluru.

The Nasscom Foundation has partnered with Rashtrotthana Parishad to get this high-end equipment installed.

In Uttarakhand, the company has deployed 122 oxygen concentrators and 125 flow meters in Cantonment General Hospital in Dehradun.

In Haryana, it has deployed an oxygen generation plant of 200 LPM (litre per minute) capacity at Faridabad District Hospital and Community Health Centre.

Besides, the company has provided nine ICU beds, four regular hospital beds, five high-flow nasal canula and 191 flow meters to the Faridabad District Hospital and Community Health Centre, he added.

“These current efforts should help in the third wave also,” Balachandran said, adding that reasonable capacity has been created in some of these hospitals to handle the likely third wave.

However, the company is keeping its eyes and ears open to meet the possible demand that may come from hospitals if a third wave hits, he added.

Nasscom Foundation CEO Nidhi Bhasin said, “The whole tech industry responded very fast. It was very overwhelming the way everybody responded, including IBM, during the second wave of the pandemic.”

The focus was on building infrastructure and creating awareness about vaccination. Companies like IBM should come forward and support these kinds of initiatives, she added.

Apart from this initiative being implemented along with Nasscom Foundation, Balachandran said IBM India is engaged in implementing CSR works in areas like education.

India-Sweden Healthcare Innovation Centre calls for covid management solutions

India-Sweden Healthcare Innovation Centre, a collaboration between the Swedish Trade Commissioner’s Office and All India Institute of Medical Sciences (AIIMS) Delhi and Jodhpur, on Monday virtually launched the second edition of the healthcare innovation challenge, calling for applications.

The innovation challenge is an opportunity for startups to collaborate with partners on Innovation Centre platform to help solve some of the problems plaguing India’s healthcare delivery landscape. This year’s healthcare innovation challenge focuses on solutions within covid management and solutions for awareness and treatment of cardiovascular, renal, neuro, cancer and lung diseases.

“Frugal innovation to develop cost-effective and efficient ways of healthcare delivery is the key need for Indian healthcare market. Innovators developing these solutions could leverage this platform and avail mentorship and guidance from our institute as well as the partners of the center to develop solutions that would benefit our country,” said Dr Randeep Guleria, director, AIIMS Delhi.

The last date for submitting applications 10 October 2021. All applications will undergo a rigorous evaluation process by a panel of experts such as AIIMS Delhi, AIIMS Jodhpur, ICMR, AstraZeneca, NASSCOM, CCamp, AIM, Invest India and various funding entities.

“Healthcare systems in India are undergoing a rapid transformation post the pandemic. Early screening and awareness tools, wearable tracking devices, remote monitoring solutions and digital tools are critical in creating a more strong and sustainable healthcare system in India. Innovators in these areas can avail the incubation facility, expert mentorship and validate the solutions at our campus through India-Sweden Healthcare innovation centre to further enhance their products and scale-up of solutions,” Dr Sanjeev Misra, Director, AIIMS Jodhpur said.

Shortlisted startups will collaborate with India-Sweden Healthcare Innovation Centre to enable faster scale up, cross-country mentorship, access to state-of-the-art incubation centre located within AIIMS Jodhpur and NASSCOM Bangalore campus, lab facilities, guidance on funding, and the ability to ideate with like-minded innovators from across India and beyond.

The first edition of the healthcare innovation challenge had seen an overwhelming response of 450+ applications from innovators and startups last year. Post a rigorous evaluation process by a panel of doctors, venture capitalists and industry leaders, 14 compelling solutions submitted by innovators were selected.

The Innovation Centre is built under strategic guidance from the Union health ministry, the Indian Council of Medical Research, Sweden’s health ministry and the embassy of Sweden in India. It also has a strong network of ecosystem partners – Atal Innovation Mission, Invest India, Startup India, CCamp, Bangalore Bio-innovation Centre, Vision Zero Cancer, and knowledge partners – AstraZeneca and NASSCOM.

75th Independence Day: 5 ways to free your mind from stress

The nation will be celebrating its 75th Independence Day tomorrow, August 15, 2021. On this day, we got freedom from British rule in India and it is a day of immense significance for us. Freedom is the power or right to do what you want without anyone stopping you, as per Oxford dictionary. When this power is snatched from us due to some reason, we feel trapped or stressed. Difficult situations in life that seem out of control may make us feel that way.

Coronavirus pandemic has made stress a constant in our lives. Freeing your mind from stress is a choice that one must make every day irrespective of the situation. If there is any resolution you should take on 75th Independence Day, it must be to say no to stress or free your mind from it.

1. Prioritise right

Over the time, if stress keeps getting accumulated, it plays havoc with our health. It’s better to nip it in the bud. Do things that are urgent and leave those that are not so important and burdening you unnecessarily. Prioritising right can take away a lot of stress that is the need of the hour.

“A lot of time we neglect our stress till the time it starts affecting us. We should address it when it is at a preventive stage. The first strategy to deal with it is to avoid stress that is not important for you,” says Dr. Arushi Malik, Counselling Psychologist, Kaleidoscope – a unit of Dr. Bakshi’s Healthcare.

2. Set your boundaries

Sometimes we think avoiding or saying no is a bad thing. But to take care and manage your stress, it is important to say no when it gets too much. “Human mind has this capability that it adapts to the situation around. One must try to overcome the thing that is stressing you out in a healthy manner. Acknowledge stress in its early stages and try to work around it,” advises Dr. Malik.

She also adds that in pandemic times, there are some things that are under our control and those that are not. “It’s important to accept the way certain things are and adapt to them accordingly,” she adds.

3. Seek help

Most importantly, do not shy away in connecting with others and seeking help. If the stress is getting unmanageable and seeping into other domains of life and impacting relationships with others, it is okay to talk about it. One must not isolate self and instead of virtual world, connect with the real world, according to Dr. Malik.

4. Do activities to soothe your mind

A lot of times it happens that stress makes you very fidgety and you are not able to focus on the task at hand.

It happens because our brain imagines the worst-case scenario. We get very jumpy and don’t focus on the moment.

Doing activities that get your composure back can be very helpful. “Doing zentangle is a fun way to draw patterns and the process is quite soothing to mind. Mandala books or colouring books for adults are also becoming popular and are very effective in soothing your mind and managing your stress,” says Dr. Malik.

These activities allow us to focus on the present moment and realise that things are not that stressful and can be managed.

5. Eating healthy and doing physical activities

Good nutrition and intake of balanced diet is important for mental health. Green leafy vegetables, fresh and seasonal fruits, nuts & legumes are rich in vitamins, anti-oxidants and other essential nutrients that help in repairing body cells and protect our brain. Brisk walking or any sort of physical activity help in alleviating symptoms of anxiety, depression, social withdrawal etc.

So this Independence Day, let’s take a resolve to kick out stress from our life and breathe.

In 6 months since launch of drive: Vaccine coverage crosses 50 cr doses

Just over six months after it launched the Covid-19 vaccination drive, India on Friday completed administering a cumulative 50 crore doses.

According to the Union Health Ministry, cumulative vaccination coverage of the country touched 50.03 crore doses, after 43.29 lakh doses were administered on Friday.

“India’s fight against Covid-19 receives a strong impetus. Vaccination numbers cross the 50 crore mark. We hope to build on these numbers and ensure our citizens are vaccinated under #SabkoVaccineMuftVaccine movement,” Prime Minister Narendra Modi tweeted.

On January 16, India launched the vaccination drive prioritising healthcare and frontline workers. As of Friday, 1.03 crore healthcare workers had received their first dose, and 79.51 lakh had received both doses. Also, 1.8 crore frontline workers had received their first dose, and 1.16 crore their second dose.

On March 1, the Centre opened up vaccination for the most vulnerable groups: people above the age of 60 years, and those above 45 years with associated comorbidities. Amid the second wave, on April 1, India rolled out a new phase, making every person above the age of 45 years eligible for a shot.

According to official data, in the 60-plus age group, 7.8 crore have got the first dose and 3.8 crore the second dose. In the 45-plus age group, 11.07 crore have got the first dose and 4.19 crore the second dose.

On May 1, India expanded its vaccination coverage to include everyone aged 18 years and above; but the 18-44 age group could get vaccinated from only 50 per cent of the doses available in the open market and procured by state governments and private hospitals. On June 21, India returned to centralised procurement of vaccines, after several states faced difficulties in procuring and managing funding of vaccines.

According to the official data, 17.23 crore beneficiaries in the 18-44 years group have received their first dose, and 1.12 crore have got their second dose. Significantly, for the 18-44 years age group, which constitutes the largest cohort, five states have administered more than 1 crore doses: Uttar Pradesh, Maharashtra, Gujarat, Rajasthan and Madhya Pradesh.

The following states have vaccinated more than 10 lakh beneficiaries of the 18-44 years group with the first dose: Andhra Pradesh, Assam, Chhattisgarh, Delhi, Haryana, Jharkhand, Kerala, Telangana, Himachal Pradesh, Odisha, Punjab, Uttarakhand and West Bengal.

On Friday, the Health Ministry said that 43,29,673 vaccine doses were given till 7 pm. “As many as 32,10,613 beneficiaries were vaccinated for 1st dose and 11,19,060 beneficiaries received 2nd dose of vaccine as per the provisional report till 7 pm. Final reports would be completed for the day by late tonight,” the ministry said.

Delhi approves policy for providing incentives to pvt sector to set up oxygen plants

Delhi battled an acute shortage of oxygen in April and May as hospitals in the capital sent out SOS calls to authorities to replenish their dwindling stocks. Several private healthcare facilities even requested the government to move out their patients.

Stepping up preparation for a possible third wave, Delhi Chief Minister Arvind Kejriwal on Tuesday approved a policy for providing incentives to the private sector to set up oxygen production plants and storage facilities to help improve the availability of the life-saving gas in the capital.

“Approved Medical Oxy Prodn Promotion Policy. It provides several incentives to pvt sector to set up oxy prodn plants, invest in Oxy tankers n set up oxy storage facilities. This will help improving oxy availability in Del which became a huge bottleneck in handling last Covid wave (sic),” Kejriwal tweeted.

On April 23, around 21 critically ill COVID-19 patients had died at Jaipur Golden Hospital in northwest Delhi due to an oxygen shortage.

Batra Hospital in Tughlakabad Institutional Area lost eight lives due to an oxygen shortage on May 1.

To prevent a repeat of the oxygen crisis, the city government is in the process of installing oxygen generation plants and increasing hospital beds to accommodate 37,000 cases a day in case of a third wave.

According to officials, around 10 Pressure Swing Adsorption (PSA) oxygen generation plants with a total capacity of 148.11 metric tonnes are being installed at various government and private hospitals in the city.

While 66 plants are being installed in Delhi government hospitals, 10 are being set up in central government hospitals and 84 in private healthcare facilities.

The city government is also creating LMO storage facilities and has floated a tender to procure 15 cryogenic tankers having a total carrying capacity of 225 tonnes.

“Free Vaccine” Campaign Covered 30-40 Crore In 24 Days: Health Minister

India’s COVID-19 vaccination coverage has exceeded 40.64 crores, according to health ministry data updated on Monday morning.

Union Health Minister Mansukh Mandaviya on Monday said it took 85 days to inoculate 10 crore, people, against COVID-19, while due to the “vaccine for all, free vaccine” campaign, it took only 24 days to reach 30-40 crore vaccination coverage.

The new phase of universalization of COVID-19 vaccination started on June 21.

The inoculation drive has been ramped up through availability of more vaccines, advance vaccine availability to states and union territories (UTs) for better planning by them and streamlining the supply chain, according to the Union health ministry.

In the new phase, the Centre procures and supplies at no cost 75 per cent of the vaccines being produced in the country to states and UTs.

India’s COVID-19 vaccination coverage has exceeded 40.64 crore, according to health ministry data updated in Monday morning.

“The world’s largest and fastest vaccination campaign under the leadership of Prime Minister Narendra Modi is continuously creating new dimensions,” Mr Mandaviya said in a tweet in Hindi.

“In the initial days, it took 85 days to vaccinate 10 crore, while due to the ”vaccine for all, free vaccine” campaign, it took India only 24 days to touch the figure of 30 crore to 40 crore,” he tweeted.

The ministry said 40,64,81,493 vaccine doses have been administered through 50,69,232 sessions, according to the provisional report till 7 am on Monday.

It said that 13,63,123 vaccine doses were administered in a span of 24 hours.

Out of the people infected since the beginning of the pandemic, 3,03,08,456 people have already recovered from COVID-19 and 38,660 patients have recovered in a span of 24 hours.

This constitutes an overall recovery rate of 97.32 per cent, which is showing a sustained increasing trend.

Maharashtra Chief Minister Uddhav Thackeray on Thursday launched the CM’s Maha Arogya Skills Development Programme, under which 20,000 youth will be trained in healthcare, nursing, and paramedical fields.

“In the next three months, 20,000 trained manpower will be provided from 36 different courses in the field of healthcare, nursing, and paramedical, which are required in Corona situation,” reads the official release.

Thackeray said the scheme will provide skilled manpower to the health sector during COVID times. “This will be an important step towards the goal of a healthy, wholesome Maharashtra,” he added.

Deputy Chief Minister Ajit Pawar, Minister for Skill Development, Employment and Entrepreneurship Nawab Malik, Industries Minister Subhash Desai, Public Health Minister Rajesh Tope, Medical Education Minister Amit Deshmukh, Chief Minister were present on the occasion.

Johns Hopkins-supported App to Guide Covid Patients at Home in India

A team of developers in India has created a new app that is supported by Johns Hopkins University in the US and designed to guide people isolating at home after testing Covid-19 positive, right on their smartphones.

The Vee+ Svasa app — developed by Bengaluru and New York-headquartered IT services company Vee Technologies, and made possible by the Valliappa Foundation — delivers educational videos, information about accessing local medical care and Covid vaccines, and other content sourced by medical experts, right to users’ smartphones.

Created by a team of software developers of Vee Technologies located in Bengaluru and Salem, the app has sourced content and expertise of medical practitioners at Johns Hopkins University and Medicine in the US, the company said in a statement.

The Vee+ Svasa was enriched by the inputs of nearly 100 teams of students from Sona College of Technology who competed for a Rs 1 lakh student prize.

The winning teams that emerged from this competition — Senthil Sivaraman S, Harsh Vardhan SM, Ragul P and Shreinik Jain U — were chosen to support the app building and running process.

“We reached out to Johns Hopkins India Institute to give access to the globally accepted standard of care protocols for home isolation cases. For Indians battling the Covid pandemic, this would be immensely helpful,” said Valliappa Foundation Trustee Chocko Valliappa.

The home quarantine app has a wealth of authentic information ranging from treating oneself at home to how to isolate oneself and protect others in the family, to tips for caregivers on measuring oxygen saturation levels as well as an easy-to-understand Covid treatment flowchart.

The information and health protocols presented in the app come only from authentic sources, such as Johns Hopkins University, the Indian Council of Medical Research (ICMR) as well as the All India Institute of Medical Sciences (AIIMS).

“We were overwhelmed by the instant response of Johns Hopkins Medicine International President of global ventures and executive VP and doctor, Mohan Chellappa and his colleagues of Indian origin, to help us gain access to the globally accepted standard of care material,” Valliappa added.

The app will also feature existing services offered by state governments like locating hospital beds, vaccination centres, and doctors on tele-consultation in key cities.

The app is available on Google Play Store while iOS version of the app will be launched soon.
